Matt, I've seen Purus's reply that this is related to your site only.  Were you able to find out what is causing the issue?

Well I'm not sure. If it is to do with my site perhaps, then It would have to be a conflict with another plugin. We do not run any custom Java script.

So no, we haven't found the problem yet. I will need to go through the plugins one by one, until we find something that conflicts.

I will install the file plugin on my dev site and see.

We have discovered that there is a possible problem with 'select2.js' and the 'Users Quick Search + Site Search' plugin.

But also, the conflict with the IM chat seems separate and is persistant without Users Quick installed - So Purus first thought seems valid......

We found a temp fix - it seems to do with jquery.metadata

When we edit out the call in ifiles - everything works as it should (except file preview of the plugin)
