Seems like you were able to resolve this.
1. I was not able to reproduce the issue with incorrect counting.
2. "Accept" button worked for me.

when we click on accept button. that button not hide after click. users gone confuse

I am having real name repeated in my site when two users are friends: user1 and user1 are now friends. {$name} = {$requester_name}. See attached image.

Alberto, can you check whether one of the users who have faced this, have privacy preferences for "view my friends" set to "Only me" or "Friends"?
