2. In debug mode, it appears in the sidebar

3. that's hard to say, we didn't notice it till after a while.

Disabling other plugins doesn't seem to make it go away....

Matt, I'm sorry is your groups plugin enabled when you see this error? if so, please deactivate/activate back the plugin and see if the issue persists.

Yes, this works. So we can assume that the method Daisy recommended does not work.
