you possibly have a js conflict, disable any new third party plugins one at a time and try it.  Also put your debug mode on.   what version are you running now ?

I disabled: Comments Advanced, Pro Blogs, Comment Editor, Event Cover , Files for Groups, File Uploads, Group Cover, Links Pro, Make Profile Picture, MailChimp Integration, News Pro, Profile Cover. And it is on the window for the changes of the label Avatar.
