No hosting is with Hostmonster. I called them and asked them to see if it was anythign on their end- and they said not that they could tell. If there is someplace they should be looking- let me know.
I am interested in a checklist. If need be I can provide login credentials as well.

David, can you please try to create two more fake roles for testing?
create two fake users and mass mail them.
create two fake users and mass mail them.
Check whether you have only one copy of e-mail on the ow_base_mail table
