If users change their avatar, then new avatar does NOT displaying in Notifications bar!

I'm sorry but this issue been fixed in 1.7.2 version. What is your activity notification version and what is your software version?

You can test it on our demo this issue been already fixed in 1.7.2.
Can you please tell us what the is the build of your activity notifications plugin?

David, I will need Cpanel access details and admin access details fo the website, as you can see there's no such issue on our demo, which is also 1.7.2 and activity notifications is 8531 as well.

Can you check that user profile? When did he change the avatar and when the comment was made and compare it to the date when you made the update?

Can you check the server error when you try to directly open the image? Maybe with tools like Firebug/Yslow for Firefox in "components" tab. Could be a cache or restriction error, too.
