I will provide a update to plugin just to verify if the issue you face is related to manual approval. The plugin new version should handle this too.We have tryed this procedure also on another website of us, and here comes the messages no more.
So the problem must be the setting.: "Mandatory user Approve", we think.?

Frankly Stan,
We are not using the SMTP possibility.
The alert of a new member, we do get.
So the plugin actually works.
Al came the alerts for a short time ago quite late.
Whether that is a problem of the server where the Web site is running, we not know so far.
But it works now again quite normal.
Really direct after the registration we still don't get it.

I really like this concept, would be nice if the emails stated which country they say they are in and which country the IP says they in in..
I have tested this plugin in couple of test instance and it works fine.
May be emails are not delivered?
