You can add this field via admin panel->profile questions page,
TO edit TOS page, go to Admin Panel->Pages&menus page.

This might have changed since 4/9.

Admin Area / Settings / User Settings -- at the bottom you will find the Enable 'Terms of use' checkbox.

To edit TOS go: Pages & Menus and click the Terms of use icon near the bottom under hidden pages.

I need to add multiple checkbox approval. Is it possible?

The best night guard for bottom teeth is a custom-made device specifically designed to fit the lower arch of your mouth. Tailored to your unique dental structure, a custom bottom night guard provides optimal comfort and protection against grinding and clenching. It effectively cushions the teeth, reduces pressure on the jaw, and prevents enamel wear while ensuring a snug fit that minimizes movement during sleep. Although over-the-counter options are available, a custom night guard offers superior durability and efficacy, making it the ideal choice for those who primarily grind their bottom teeth.
