you can not just add question in the database you need to do it in admin profile questions

From admin panel go to Users > Profile Questions. There you can create a profile question of any type easily. Do not have to put anything to database.

The question are store in ow_base_question.
The text for those question is located in the language file.
See the name field. You will find some value with a format like:
"7e93e75b8b41706d78017b4146cf2530"
This will be link to a key in the language file where the text for the question is located.
